Kurzfassung:
The widely accepted DICOM standard (Digital Imaging and Communication in Medicine) provides a set of protocols that facilitate the communication of cardiac digital images and related data across a network (Internet) in an open vendor environment. This standard however does not specify how such studies (images) should be archived or managed. The first part discusses the design of a prototype DICOM Hypermedia Video Server (DHVS). The DHVS, which is based on the Hyperwave Server Technology, will provide efficient archive, query, retrieval and management functionality in a distributed cardiology environment. It thus will offer both time and cost benefits in terms of research, administration and education in digital echocardiography. The second part investigates the diagnostic accuracy of MPEG-2 compressed echocardiograms. Because of the large volume of data involved in cardiac digital imaging, digital compression becomes inevitable in archiving and communicating such imaging studies. For this reason a new methodology for evaluating the diagnostic accuracy of medical imaging systems was developed (AFROC: Alternative Free Response Receiver Operating Characteristics) and a clinical experiment was conducted for the first time worldwide at the Harbor UCLA Medical Center (University of California Los Angeles) based on this methodology. The new methodology, study design, experimental procedures and results are presented.
